Medical Forms & Waiver Form

All participants must have their own personal medical insurance. All medical emergencies and illnesses are referred to UMass Health Services and/or Cooley Dickinson Hospital.

To limit our amount of paperwork, please submit our medical form ONLY.

Parent/Guardian must complete in detail the medical and waiver form and sign for permission for any needed medical attention

All necessary paperwork is due no later than 1 week prior to the start of the academy.

ALL INCOMPLETE FORMS WILL BE RETURNED.

Any medication(s) whether over the counter or prescription will be collected at the start of each day by our Athletic staff trainer for protection and administering.

Athletic staff trainer will be on duty to deal with athletic injuries but do not dispense painkillers. Those who require taping should indicate so on the health form.
